New River Valley Agency on Aging (NRVAA) provides an assessment of the older person's needs and works with the individual and/or their caregiver to identify and obtain services, equipment, information, etc. This service is provided by human service professionals. Persons who receive care coordination services are monitored through home visits and telephone contacts to determine if their needs are being met. There are different levels of care coordination according to the level of care needed.
