Heritage Green Assisted Living Community in Lynchburg offers Daybreak, a campus for people with various degrees of memory impairment and Alzheimer's disease. The Daybreak program for the memory impaired is a care delivery system based on a multi-departmental approach and includes the phases of assessment, planning, intervention, and evaluation.
Daybreak Memory Care Program is a structured, 7-day-per-week program of care with a holistic approach that integrates the psychosocial and medical models into an activity-focused model to include all events that occur during the day of a resident with dementia.
Family involvement through the Care Conference, is a structured process, for the coordination of care, communication, and collaboration. Family support is provided through education, emotional support, and the opportunity for involvement in the program of care. Staff support is provided through specialized training in dementia care, team building, and customer service.
