Guardianship

Chesapeake Human Services Department is a state-designated Guardian and offers guardianship and conservatorship services to an individual who requires a surrogate decision-maker but has no other appropriate individual in their lives or resources to serve in this capacity.

The agency may assume a variety of decision-making roles, depending on the individual's needs, including guardian of the person, conservator, representative payee, medical power of attorney, durable power of attorney and authorized representative for people in state facilities.
